AR-CoC-f - Analytical Request / Chain-of-Custody
form, for use when submitting samples. This form is interactive. It can be
filled in on-screen, saved to disk, and printed.
AR-CoC-i - Instructions for using the Analytical
Request / CoC form. Also includes our test list with the corresponding test
codes.
Collection Rpt - Sample Collection Report form, to be
used for FDA DWPE samples (automatic detention samples).
Test List - Abbreviated listing of all tests currently
offered by Columbia Food Laboratories. Please contact us for test specifics.
We have USDA import permits available for those customers wishing to send us
soil or vegetation (raw, non-processed) samples from outside the United States. Having the proper
permit attached to the package facilitates passage through US Customs. Please
call or e-mail us for details.
